Candace Holder,
Memory Care Coordinator
As the Memory Care Coordinator, Candace is responsible for creating environments in memory care where moments of Joy, Independence, and Wellness are the focus each and every day. She trains, motivates, and evaluates caregiving staff to ensure that the highest quality of care is provided for residents. She assists the Health Services Coordinator with assessments and service plans. Candace uses this information in collaboration with the Life Enrichment Coordinator to plan meaningful activities. She also performs the duties of a Resident Assistant or Medication Aide.
Candace has her CNA license, training in Administrative duties, and special training to care for elders with Alzheimer’s and dementia. She came to Prairie House in February of 2000, as a Medication Aide. She then moved to the position of Memory Care Coordinator once the Memory Care was built at Prairie House.
Candace is married and has 4 boys and 3 step children. Camping, fishing, and spending time with family and friends are her favorite pastimes. She provides care for an elderly aunt that lives in their home. Candace’s compassion for care giving came from her family who were all active in care giving. She also volunteers for the American Legion, where the focus is on veterans, their families, and the community of La Pine. Putting her heart and soul into the care she provides for people both at Prairie House and in the community, is what Candace does best, making her such a valuable part of our family here.
